Pathway 1: Start Your Case
Provide a free‑form description of the medical situation you would like help with.
You may be as brief or as detailed as you wish.
- For longer descriptions, we recommend writing your summary in a separate document and attaching it along with your other materials (see #3 for details).
We understand that much of your case can be reconstructed from the clinical notes you provide.
However, a first‑hand, detailed description from the patient or a family member is often the most valuable source of information.
- Medical notes are frequently copied and summarized across multiple consultations, which can lead to the omission of important details.
- This is often done for the sake of brevity or to fit a unique patient story into standardized clinical language.
Your personal account helps preserve context, nuance, and what truly matters to you.
We can work on a case even if you do not submit specific questions.
However, it is important that we address all questions that matter to you, and that we understand your goals for the case. Patients with similar medical situations may have very different priorities.
- For this reason, it is most helpful if you provide a complete list of questions you would like addressed.
- Please include brief clarifications or background information wherever you feel it would help us better understand the context of your questions.
We work with clients from around the world and have learned not to dictate the scope, format, or method of data transfer.
Transfer of Medical Reports
You are free to share your documents in the way that works best for you.
- You may send us documents in any language — no translation is required.
- You may redact or remove any personally identifiable information. We only need an approximate age and sex to create a clinical case for expert review.
We recommend gathering as many relevant documents as possible. These typically include:
- Physician visit summaries
- Consulting notes
- Hospital discharge summaries
- Laboratory test results
- Imaging reports (CT, MRI, PET)
- Reports from other diagnostic tests (e.g., angiography, ultrasound, EEG, EMG, Holter monitoring)
Please scan your documents with high quality and save them in PDF format.
- If scanning is not possible, take clear photos and convert them into PDFs.
- Whenever possible, avoid handwritten notes, as they are often difficult to interpret.
Once you’ve gathered and scanned all your documents, place them in a single folder and upload it to a file transfer service you prefer and send us a download link.

Once we confirm that we have successfully downloaded your files, you may (and should) delete them from the file-sharing service.
Transfer of Imaging Studies
In many cases, experts will need to review your actual imaging studies (e.g., MRI, CT, PET-CT, angiography). These are typically provided to you:
- On a CD/DVD
- As a download link in the original DICOM format
Please follow these steps:
- If you have a CD, transfer the entire contents of the disc into one folder on your computer.
One study = one folder.
Do not mix multiple studies in the same folder. - If you receive a download link, save the study into its own separate folder on your device.
- You may compress (zip) the folder before uploading it to a file transfer service.
If you prefer to anonymize your imaging studies, you can do so yourself (or ask a trusted IT professional to help) using the free desktop imaging viewer HOROS.
- Simply import the imaging study into HOROS, then click the “Anonymize” button. This will save an anonymized version of the study to your computer, which you can then send to us.
Send us the download link for each imaging study.
Again, one folder = one study = one download link.
Please also upload the written report for each imaging study along with your other medical documents.
